Practice Charter


The practice complies with data protection and access to medical records legislation.  Information about you will be shared with others only in the following circumstances.


1)  To provide further medical treatment for you e.g. from district nurses or hospital services.


2)  To enable you to get help from other services such as Social Workers, Occupational Health etc. Your consent is required for this.


Administration and reception staff need to have access to your records in order to do their jobs.  They are bound by strict rules of confidentiality.

Freedom of Information

Information about the General Practitioners and the Practice required for disclosure under this act can be made available to the public.  All requests for such information should be made in writing to the Practice Manager.

Access to Records

In accordance with the Data Protection Act 1998 and Access to Health Records Act, patients may request to see their medical records.  Such requests should be made through the Practice Manager and may be subject to an administration charge.  No information will be released without the patient consent unless we are legally obliged to do so.


The NHS operates a zero tolerance policy with regard to violence and abuse, and the practice has the right to remove violent patients from the list with immediate effect in order to safeguard practice staff and patients.  


Violence in this context includes actual or threatened physical violence or verbal abuse which leads to fear for a person's safety.  In this situation we will notify the patient in writing of their removal from the list, and record in the patient's medical records the fact of the removal and the circumstances leading to it.


We understand that people can behave differently when worried, unwell or under stress, but it is not part of our job to endure abusive behaviour, either verbal or physical.


We have the right to remove from our list any patient whose own behaviour, or the behaviour of their relatives is unacceptable. In this situation we would normally issue a written statement explaining why this decision has been reached.

The practice adheres to the following policies

We are pleased that you have chosen us to look after your health, but you should remember that doctors, nurses and other health workers are not solely responsible for maintaining your health status.  You have a responsibility to yourself to take appropriate measures for improving and maintaining your health wherever possible, and this may include the following advice.

Whenever possible please try and book a routine appointment to discuss health problems or concerns.


Urgent problems will be dealt with on the day of request.  Please do no abuse this system with long-standing problems, minor ailments or forms to be filled out (i.e. certificates, travel documents, etc) because this will overload the system, overwork the doctor and nurse and make other patients wait longer

Home visits should only be requested by housebound patients or those genuinely too ill to attend the surgery.  Requests for a home visit should be made by no later than 11.30am where possible.

Calls outside surgery hours and especially at night can often be avoided, except in cases of genuine emergencies.  If you are thinking of calling the surgery, please do so during surgery hours and don't delay until we are closed.

If you cannot keep your appointment do not fret as there are always others who can, but PLEASE inform us as soon as possible so that the appointment can be offered to somebody else and then no time is wasted.

Our philosophy is to treat all patients with equal respect.  We have zero tolerance of any verbal or physical abuse towards any member of staff.

It is our policy to respect the privacy and dignity of our patients.


If you would like a chaperone to be present during a physical examination/consultation or if you would prefer to  be examined by a health professional of the same gender as yourself, please let us know and we will do our best to comply with your wishes.
